The Age-Friendly Long-Term Services and Support (LTSS) ECHO series, which is scheduled for the 3rd Thursday of each month, from 2 to 3:15pm, will begin on Aug 20, 2020.  Following the Project ECHO Model^TM, the UGEC Learning Community is transitioning to the Age-Friendly LTSS ECHO with Comagine Health’s support. 

The Age-Friendly LTSS ECHO Model^TM builds on the prior UGEC Learning Community, connecting community providers in collaborative learning with specialists by Zoom conferencing.  The ECHO model includes case-based learning and mentorship to help interprofessional teams gain the expertise required to provide needed care and/or services. Sessions will begin with introductions, followed by a 15-20 minute didactic, 1 or 2 case presentation(s) and a discussion. You’ll also receive a form for submitting cases.
